Index entries remaining separate, not merging

After embedding my index, some of my entries are not merging but remaining on separate lines:

neo-liberalism, 306, 311
neo-liberalism, 403
neo-liberalism, 417
neo-liberalism, 417
neo-liberalism, 418

(Pages 300+ are in chapter 3, pages 400+ in chapter 4. This is a multi-file Word project).

I thought at first the problem might be to do with different fonts in the XE tags, but no other entries are affected, even when they similarly are from chapters 3 and 4.

Does anyone know what could be going wrong?

The problem seems to have been that I also had an entry for ‘neoliberalism’ – i.e. without the hyphen. Once I put a hyphen into that, all the entries merged correctly.

Why the presence or absence of a hyphen in one entry should affect the others, I have no idea – but at least it works.
